​Federal Application Requirements

All potential agency applicants and sub-recipients for federal funding must have a Unique Entity Identifier (UEI) and be registered with the System for Award Management (SAM). In addition, the state requires applicants to register as a Commonwealth Vendor.

Getting a UEI number

If your agency had a DUNS number and was registered in SAM prior to April 4, 2022  your agency was assigned a UEI number.  If your agency did not have a DUNS and SAM registration prior to April 4, 2022, please go to SAM.gov | Entity Registrations to request your UEI. SAM registration status can be checked at SAM.gov | Home. If you need assistance with this process, call SAM Customer Service at 1-866-606-8220 or use the live chat help on the SAM website.

Registering with SAM

Once you receive your UEI, go to the SAM System and click “Get Started”. Registration should take 10 minutes to complete. Click “Create an Account” on the right of the page. Allow 1-2 days after completing the forms to receive your valid registration.

Government Performance and Results Act (GPRA)

Agencies applying for grants under State Opioid Response (SOR) funding should be aware of the GPRA collection requirement. For more information regarding GPRA, scroll to the bottom of the For Single County Authorities page.

Registration as a Commonwealth Vendor

In order to receive subgrant funds from DDAP, the applicant agency must have a valid Commonwealth Vendor ID number. If the applicant agency does not have a Commonwealth Vendor ID, the applicant agency may register for one on the Commonwealth of Pennsylvania’s Vendor Registration website.
