Reporting Changes

Drug and Alcohol Treatment Facilities

Changes of ownership (CHOW) may occur at drug and alcohol facilities licensed by the Department of Health.  28 Pa. Code §§ 709.14(b)(1) and 711.14(b)(1) require that the Division of Drug and Alcohol Program Licensure be notified of any ownership changes within 90 days of the occurrence.

New owners are required to submit a signed Ownership and Business Management Form.

Recovery Houses

Licensed recovery houses must notify the Department in writing any time the house manager changes within 30 days.

Recovery houses licensed by DDAP must notify the Department within 90 days of the occurrence of any of the following conditions as specified in § 717.9 (relating to restriction on license):

  1. Change in ownership.
  2. Change in name of the drug and alcohol recovery house.
  3. Change in location of the drug and alcohol recovery house.
  4. Change in maximum occupancy.
  5. Closing of the drug and alcohol recovery house
Failure to notify the Department as required under subsection (b) will result in automatic expiration of the license.

Change in House Manager
New House Managers must submit a signed House Manager Information form.

Change of Ownership
New owners must submit a signed Recovery House Ownership and Business Management form.

Change in Name, Location, or Maximum Occupancy
House Managers must submit a signed Recovery House Information Change form.

Recovery House Closure
House Managers must submit a signed Recovery House Closure form.
